Job Description

Tri-State Rehabilitation is seeking Full-time/Permanent, Contract, and Part-time Occupational Therapists for a Skilled Nursing Facility.


The Occupational Therapist is responsible for evaluating, directing and administering of Occupational Therapy modalities of treatment. The Occupational Therapist administers treatments in an effort to restore function and prevent disability following injury, disease, or physical disability. The goal is to assist patients to reach their maximum performance and level of functioning, while learning to live within the limits of their capabilities.

Job Responsibilities
• Develops and implements a treatment plan designed to meet the stated goals. Periodically assesses the effectiveness and adapts treatment program based on technical skills and clinical judgment.
• Accurately and completely documents patient evaluations, progress notes, discharge summaries and other forms necessary to communicate patient status and to receive payment for services rendered.
• Contributes to department productivity through effective time management of patient treatment and documentation.
• Coordinates, delegates, and supervises responsibilities assigned to supportive staff (COTA).
• Consistently reviews and modifies goals with the patient and appropriate caregiver.
• Establishes short and long term goals based on the therapy evaluation and interacts with members of the health care team, the patient, and the family/caregivers regarding care, functional capabilities, and progress toward goals.
• Accurately instructs the patient/caregiver in all equipment use.
• Completes all patient evaluations and develops the plan of care within state specific guidelines. Reports outcomes of evaluation, goals, and anticipated projected frequency of care to a supervisor within 24 hours of the evaluation.
• Performs and prioritizes multiple functions or tasks to effectively deal with multiple changes based on patient need and scheduling.
• Prepares and submits legible, relevant and sufficient documentation - whether written or electronic - of treatment and skilled intervention provided, as well as report of a patient’s progress toward goals in accordance with professional standards of practice, policy and procedures, and other requirements.
• Participates in staff conferences and committees as necessary.
• Provides in-service education as requested by Director or Rehab Manager.
• Participates in quality and performance improvement measures
• Complete other assignments as requested and assigned.

Job Requirements

• Must be a graduate of an accredited and approved Occupational Therapy educational program.
• Must have a valid state license to practice Occupational Therapy.
• Current Driver's License and vehicle insurance, and access to a dependable vehicle.
• Current CPR certification required.
