Job Description

Tri-State Rehabilitation is seeking Full-time/Permanent, Contract, and Part-time Physical Therapists for a Skilled Nursing Facility.

The Skilled Nursing Physical Therapist is responsible for the assessment and evaluation of patient care needs related to functional status/mobility, endurance/stamina, pain and other medical conditions. Based on this assessment and evaluation, the Physical Therapist works to help determine a treatment plan, performs interventions aimed at improving and enhancing the patient's well-being, and evaluates the patient's progress.

Job Responsibilities
• Reads and interprets technical instructions related to the care of the patient and use of equipment in providing such care.
• Visually and aurally observes and assesses the patient.
• Treats patients to relieve pain, develop and restore function, and / or maintain maximum functioning using acceptable standards of practice.
• Assesses changes in the patient’s status since the last treatment day, and determine whether the planned services should be modified, performing a re-evaluation and obtaining revision orders as indicated.
• Instructs and informs participating members of the health care team, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
• Visits patients according to Plan of Treatment, completes a progress note for each visit, and submits progress notes to the agency at least twice a week.
• Completes all patient evaluations and develops the PT plan of care within state specific guidelines. Reports outcomes of evaluation, goals, and anticipated projected frequency of care to a supervisor within 24 hours of the evaluation.
• Performs and prioritizes multiple functions or tasks to effectively deal with multiple changes based on patient need and scheduling.
• Prepares and submits legible, relevant and sufficient documentation - whether written or electronic - of treatment and skilled intervention provided, as well as report of a patient’s progress toward goals in accordance with professional standards of practice, policy and procedures, and other requirements.
• Participates in staff conferences and committees as necessary.
• Provides supervision of the PTA as per Tri-State policy.
• Prepares a written discharge summary and/or a written summary report on patient's condition.
• Provides in-service education as requested by Director or Rehab Manager.
• Participates in quality and performance improvement measures
• Complete other assignments as requested and assigned.

Job Requirements

• Must be a graduate of an accredited and APTA approved Physical Therapy educational program.
• Must have a valid state license to practice Physical Therapy.
• Current Driver's License and vehicle insurance, and access to a dependable vehicle.
• Current CPR certification required.
